TATTOO CONSULTATION REQUEST FORM
Please fill out this form to request a date and time to collaborate about your tattoo IN PERSON, at the shop!!! We can discuss the design, style, placement on the body, budget and an estimated price for your tattoo.
We will figure out a date and time that works for both of us to start your project!!!
*Must be 18 years old to get tattooed at Mr. Blue Sky Tattoo.
( we do not tattoo minors under any circumstance)
*Identification (drivers license, state issued ID) is required to get tattooed.
*Our minimum charge for a tattoo is $100.
*A DEPOSIT OF $50-$200 (depending on the size of the tattoo)
IS REQUIRED AND NON REFUNDABLE UNDER ANY CIRCUMSTANCES. CANCELLATIONS OR RESCHEDULING LESS THAN 7 DAYS BEFORE YOUR APPOINTMENT WILL RESULT IN LOSS OF DEPOSIT.
NO CALL NO SHOW WILL RESULT IN A LOSS OF DEPOSIT AND AN AUTOMATIC BAN FROM THE SHOP. Please respect our time.
*One shot, single session tattoos are priced accordingly at a flat rate.
*Our hourly rate for large, multiple session tattoos are $200 an hour with a minimum of 3 hours per session.
*We do not tattoo fingers, the inside of lips, genitals, or faces.
*We do not tattoo racist or gang affiliated imagery or words.
*Children or anyone under the age of 18 are NOT PERMITTED inside the tattoo shop.
All forms will be read and answered as soon as possible.
Please include the style of tattooing you would like your project to be as well as approximate size and placement on the body. Please upload any reference pictures.
